(“LiveWell” or “we” or “us”) collects from you when you use our website, LiveWell.org (“Website”), interact with our third party partners (“Partners”), or interact with us directly on our Website. If you decide to register for seminars, make a donation, or purchase our educational material, we’ll collect information from you to facilitate your request, including you name, address, and other information as applicable to your request.

If you decide to fill out a LiveWell application for services or residential care, you will be providing extensive personal information about yourself, your family, your physical health and condition, and your finances. We will use this information to determine your eligibility for LiveWell services, including community services, adult day services, assisted living services, and skilled nursing care.

Website Visitor Data
We collect visitor information, including your IP address and server log data (the address of the web page you visited before using the Website, your browser type and settings, the date and time of your use of the Website, language preferences). We may gather your information about the device you are using to access our Website, including what type of device it is, what operating system you are using, device settings, application IDs, unique device identifiers, and crash data. Other data is collected, including data generated by your use of the Website, support communications with us and links you interact with. We may also determine location by using other data from your device, such as precise location information from GPS, information about wireless networks or cell towers near your mobile device, or your IP address.

Health Information
Some health and/or health-related information that you elect to share with us via our Website may be protected by federal and/or state laws. We only use and disclose such information as permitted under the law and as set forth in this Privacy Policy.

We may use your information to register you for seminars, facilitate your purchase of educational materials, or assist you with making a donation.
We may use your information to evaluate your eligibility for LiveWell services.

Do-Not-Track
You may have implemented a “do not track” signal through your browser. As there currently is no fixed standard for do not track signals, we currently do not respond to do not track signals from your web browser.

Data Retention and Destruction
We will retain Personal Information for as long as necessary to accomplish our purposes for such data as set forth in this Privacy Policy. You can request that your account be deleted by contacting us at the address provided at the bottom of this Privacy Policy. At the time your account is deleted, we will destroy your Personal Information using reasonable data destruction practices. We may, however, retain certain information to comply with legal or contract obligations or to facilitate law enforcement requests.

California Privacy Rights
If you are a California resident, you may ask us for a notice identifying the categories of Personal Information that we share with our Partners and/or third parties for marketing purposes and to refrain from sharing your Personal Information with certain of these entities for their marketing purposes.
