LiveWell is the only Occupational, Physical and Speech Therapy provider in Connecticut to have a dedicated practice for people living with dementia and their families. Our team of dementia specialists can be trusted to help you develop the adaptive strategies you need to compensate for the cognitive and physical changes experienced with dementia. We work with individuals with dementia, family members and care partners to strengthen retained abilities, promote independence and maximize quality of life.

OCCUPATIONAL
therapy Services
LiveWell’s Occupational Therapists work exclusively with people with dementia and their family members. We assess function and cognition to identify and maximize retained abilities. Evidence based caregiver services including COPE (Care of Persons in Their Environment) and Skills2Care are also available for those struggling with behavioral expressions. Services are offered at LiveWell and to people at home under Medicare and private insurances.
Physician Specialists
& Palliative Care
Accessing a Geriatrician who specializes in dementia can provide clarity and answers for families seeking a diagnosis, managing symptoms, and managing medications. Our Palliative Care Physician can also guide discussions on your goals of care, end of life choices and advance directives. Physician services are offered at LiveWell and to people at home under Medicare and private insurances (fees may apply).
Staying engaged
It is easy to become isolated and disengaged from friends, family and meaningful activities when daily living becomes increasingly challenging. Staying connected and engaged becomes all the more important for both a person living with dementia and their caregivers. LiveWell offers several dignified opportunities for people with dementia and their families to stay connected with others and engaged in meaningful activities. Unique offerings are available for people at various stages of their dementia experience.
community Connections
A weekly excursion program for people beginning to experience early memory changes. Opportunities exist to discuss dementia symptoms, build relationships, explore interests and pursue passions through classes, workshops, arts, cultural, social, and wellness opportunities.
Day CenteR
Our social model day program is exclusively for people experiencing dementia. Vibrant experiences focus on wellness, learning, and creativity. Members get connected, engage in community and thrive. Regular outings include trips to the shore, local wineries, fitness centers, and local volunteer opportunities. Full day, half day, extended day and individual a-la-carte program options are available in Southington and transportation is available within a regional radius.
memory cafes
A monthly social gathering for people living at home with their spouses, partners and families. Cafes provide an opportunity to meet with others who can relate to your experience in a welcoming and comfortable atmosphere. Adjusting to a dementia diagnosis can be emotionally challenging. Left untreated, depression, anxiety and other mental health conditions can add to an already challenging situation. Our Licensed Clinical Social Workers offer specialized counseling services to people living with dementia, family caregivers and friends. Specialty groups include MBCT (mindfulness based cognitive therapy) and MBSR (mindfulness based stress reduction). Services are offered at LiveWell and to people at home under Medicare and private insurances.

HOME SAFETY & TECHNOLOGY SOLUTIONS
As cognition and physical abilities change with dementia, a person’s home environment can either be helpful or challenging. New safety concerns will arise and environmental modifications can have a positive impact. Technology solutions can also increase safety, engagement and help families coordinate care. LiveWell’s Occupational Therapists provide home safety and technology assessments and help families find solutions to maximize safety and independence for those living with dementia at home.
